During an in-home assessment, a specialist will arrange either an in-person visit or a video conference call to evaluate the suitability of our facility for your loved one. During this session, we aim to gather comprehensive information about your loved one using a comprehensive range of tools to assess the level of assistance they require. We understand that making this decision can be emotionally challenging, and we’re here to provide support at every step of the process. We assist with the moving day and offer counseling for family members to navigate this transition as their loved ones settle in and beyond.
If we determine that your loved one may not be quite ready for our facility, we work closely with you to establish an estimated timeline and create a plan for “when the time comes.” We provide guidance on in-home care options and collaborate with you in advance so that when the moment arrives, you’ll feel comfortable having your family member join our community and feel right at home.
We view the in-home assessment program as a significant step forward in the integration process for your loved one. It’s another example of our forward-thinking approach that sets One Kenton Place apart.
Our thorough on-boarding program follows a personalized resident-centered care approach to encourage a smooth transition.
Our comprehensive onboarding program adopts a resident-centered care approach tailored to ensure a seamless transition. Here’s how we facilitate your loved one’s adjustment:
Before moving in, we offer pre-residency visits, allowing your loved one to acquaint themselves with life at One Kenton Place. This is highly encouraged as a way to familiarize yourself and your loved one with our atmosphere and daily life. You and your loved one can participate in welcome events and introduction programs, enabling them to meet fellow residents and our dedicated staff and develop a level of comfort.
During the initial days of residency, we conduct a range of assessments, including medical, dietary, cognitive, participative and fall risk evaluations. In the first two weeks, each new resident undergoes a heightened observation regimen. This helps us gain deeper insights into their medical and psycho-social needs. Furthermore, we take the time to explore your loved one’s life experiences, family history, as well as their interests and passions. We offer a diverse array of life skill and leisure programs, tailoring our approach to align with their interests, whether it be music, playing an instrument, or a love for chess, to ensure they feel at home.
We encourage families to personalize the resident’s suite with meaningful items like photos, artwork, mementos, and memory boxes, fostering a sense of familiarity. Residents are also seated with others at their respective tables, promoting early friendships and connections.
We maintain open and ongoing communication with family members, providing regular updates on their loved one’s progress. If deemed safe, residents are encouraged to continue visiting their family homes for short stays, special meals, and occasions, to ensure they remain connected to their roots.
We actively promote family and friends’ involvement in the life of One Kenton Place. Whether it’s enjoying a meal together, spending quality time visiting, or going on outings to local restaurants and shops, the positive impact on loved ones is immeasurable.
Families are an integral part of our Care Team, which distinguishes us from other assisted-living facilities. While we become well-acquainted with your loved one, you will always be the foremost expert on their preferences and needs. We don’t enforce set visiting hours, and you’re welcome to visit as frequently and for as long as you like, as long as it doesn’t disrupt the care provided to your loved one or other residents. We also invite you to take your loved one on outings as often as you wish. When it comes to family, “the more, the merrier” – feel free to bring your children, grandchildren, or even the family pet.
Furthermore, we have the capability to host special events within our facility, whether it’s a birthday celebration, an anniversary, or a simple Friday night dinner. We can assist with planning and catering for these occasions. Families are also invited to join us for complimentary Sunday brunches with live music, as well as educational evenings featuring guest speakers once a month.
During the initial days of your loved one’s stay, we encourage families to help ease the transition into our home by spending time with their loved ones. We understand that the first day can be overwhelming for both the resident and their family. To assist with this transition, we recommend that families share a delicious meal together proudly offered by our outstanding culinary team before commencing the intake process. We believe that “breaking bread” with family sets a welcoming tone and serves as a pleasant introduction to our community.
Once your family member is settled into our facility, you’ll find that many of the stresses and anxieties associated with their care prior to their move have drifted away. We enjoy watching families once plagued by uncertainty and fear of the unknown be able to focus on what matters – time with their loved one – now that their care is secure and they are in an ideal environment to thrive.
